Restaurant worker Jose Rodriguez, who is standing in a crowd of people, is apparently hit on the knee as the car goes past.
The a 34-year-old from Jersey City, New Jersey stays on his feet and walks after the vehicle as it moves away.
He later called emergency services and was taken to hospital for treatment to his knee.
Rodriguez, who had just finished his shift at the nearby Maritime Hotel, has hit out at Lohan and her entourage.
"They acted like I was nothing," he told the New York Daily News. "That no one could touch her because she was so rich and powerful."
The 26-year-old actress was later arrested as she left the nightclub at the Dream Hotel in Manhattan's Chelsea neighborhood and given a ticket.
She is due to appear in court on October 23.
Mr Rodriguez claimed Lohan had been "slurring" her words but police said there was no evidence she had been drinking.
The star's publicist dismissed the incident as "much ado about nothing" and said he was confident she would be cleared.
